NOTE: – The UKRAINE tourist or business visa is issued for a single entry or double entry within six months of issue date, for the dates of entry and exit not to exceed those stated on your application form and invitation or confirmation. The usual period of stay does not exceed three months, but upon application to a local OVIR (registration office), extension is possible for a maximum stay of six months.
– Invitations from church organizations must be stamped by the city registration office, or the Ministry of Religious Affairs in Kiev.
– Double entry visas can be issued if travel itinerary specifies two entries or if a letter is attached to the application requesting a double entry and explaining why it is needed.
– Invitations must bear the seal of the issuing company or organization
Passport
Please send your current valid and signed passport. U.S passport must be valid 6 months beyond intended stay and must have at least one blank visa page. Foreign Embassies cannot issue a visa on a page not labeled as a Visa page. Amendment and Endorsement pages in the back of the passport cannot be used for issuance of a visa.

Proof of Departure
Please send a copy of your round trip airline tickets, or itinerary or e-ticket, or letter of confirmation from a travel agent. The itinerary must show the applicant’s name, date of arrival and departure from the country, and flight information. The Embassy does not recommend purchasing your ticket prior to a visa being issued.

A. Tourist Voucher. An Original tourist voucher issued by a Ukrainian travel agency (Tourist visa)
B. Private visit. An Original invitation issued in the name of the applicant from a local Immigration Authorities of the Ministry of Interior of Ukraine (Private visit visa). - Non-US citizen A Photocopy of Permanent Resident Card (Green Card) or valid U.S. visa and I-94 is required.
Travelers who entered the USA after April 26th 2013 should print the electronic I-94 form from the Department of Homeland Security. - For Private Stay Letter of Invitation issued by the Ukraine Ministry of Interior excepting for citizens of the USA.Canada, Japan, Switzerland, Slovakia and Turkey, or for citizens of counties of the European Union.
- Health insurance. Proof of international health insurance specifically covering travel to Ukraine.
- Bank statement. Copy of the applicant’s most recent monthly bank statement. The statement must clearly show the applicant’s name as the account holder, the balances of the accounts, and the date of the statement.
BUSINESS / RELIGIOUS/HUMANITARIAN VISIT
- Computer generated flight itinerary from the travel agent
- ORIGINAL Letter of invitation from the host company in Ukraine (must bear the seal of the issuing company or organization)

- For Humanitarian aid Visit A visa is issued on the basis of a letter of support from the Committee on Humanitarian Aid of the Cabinet of Ministers of Ukraine. If an applicant provides the Consular Office with a letter from the Committee on Humanitarian Aid of the Cabinet of Ministers of Ukraine, then the visa will be issued free of charge (application and visa fee are waived.)
- Health insurance. Proof of international health insurance specifically covering travel to Ukraine.
- Bank statement. Copy of the applicant’s most recent monthly bank statement. The statement must clearly show the applicant’s name as the account holder, the balances of the accounts, and the date of the statement.
